You are still welcome to see any of our providers as a Self-pay patient.
That depends upon your insurance plan. We suggest you call your PCP to notify them of your appointment with us and request they process an insurance referral if necessary.
Most insurances require some type of co-payment. We suggest you contact Member Services by using the phone number located on the back of your insurance card to find out. Please understand that co-payments are due at the time of each visit.
We do not take walk-in appointments, however, we urge you to give our office a call first. There is a good chance we can get you in on the same day if you are having an urgent issue.
Yes, it’s actually very easy! Click on the “Make a Payment” link on our home page. You will be directed to our patient portal where you can sign in, view your latest statement and pay your bill. Not sure if you have an account or don’t know your username or password? No problem, simply click on “Trouble logging in” for help recovering your account information.
The patient portal has a limited view of your medical records. You are able to view/update your personal info, pharmacy info, allergies, and past surgical history. If you need to see your complete medical records, simply click on the “Request for Medical Records” link on our home page and fax the form as instructed.
Each patient is assigned a primary Physical Therapist who conducts your evaluation and determines your treatment plan. We also team you up with a Physical Therapist Assistant who works closely with your primary therapist to carry out the treatment plan and ensure there are no gaps in your treatment schedule.
Sports Medicine is a specialty practice, providing care for athletic and recreational injuries sustained by people of all ages. Today’s athlete can be a little league baseball player, a working mother interested in staying fit, or a senior citizen who golfs on the weekend. These people, in addition to high school, collegiate, and professional athletes, will benefit from the care of a sports medicine professional. Through advances in surgical techniques, orthopedic sports medicine specialists have been able to treat and rehabilitate athletes whose types of injuries were once career-ending. In addition to improved surgical techniques, sports medicine has also made tremendous strides in identifying, treating and preventing sports-related injuries through nonoperative care.
